Expert Video Report on combination therapy in advanced prostate cancer - ASCU GU 2022

Reporting from the ASCO Genitourinary Symposium 2022, Fred Saad provides an overview of practice changing studies with combination therapy in advanced prostate cancer. He features the improved rPFS demonstrated in PROpel and MAGNITUDE with a combination of PARPi with abiraterone or AAP in patients with mCRPC. He also discusses reduction in the risk of death by adding darolutamide to ADT + docetaxel in patients with mHSPC and other endpoints fovouring the triplet therapy.

Reporting from the ASCO Genitourinary Symposium 2022, Petros Grivas comments on data from studies that support further evaluation in urothelial cancer, in particular sacituzumab govitecan + pembrolizumab in the metastatic setting for platinum-refractory disease, nivolumab + trastuzumab deruxtecan in patients with HER2-expressing tumour, and single agent enfortumab vedotin in the neoadjuvant setting before radical cystectomy in patients who are cisplatin-ineligible.

He begins by covering the informative findings from the ARCHES study testing enzalutamide plus ADT in hormone-sensitive prostate cancer. Then he comments on results from the castration resistant setting in the COSMIC-021 trial testing a combination of atezolizumab + cabozantinib, and finally, discusses results from the SAKK 08/16 study about darolutamide maintenance therapy.

Reporting from the 2021 Genitourinary Cancers Symposium, Karen Knudsen comments on results from CRPC studies looking at molecular determinants associated with long term response to apalutamide in the SPARTAN study, and the final results from the ACIS study testing the addition of apalutamide to abiraterone and prednisone in metastatic disease.
She also gives an overview of the final results from the TITAN study testing apalutamide in metastatic castration-sensitive setting receiving ADT.

Reporting from the ESMO Virtual Congress 2020, Maria De Santis comments on promising therapeutic options and practice changing results presented in metastatic castration-resistant prostate cancer (mCRPC).
She emphasises the importance of testing CRPC patients for genomic alterations (BRCA1, BRCA2 or ATM) as soon as the cancer metastasises, to identify those who could benefit from targeted treatment options.
