Background
Third-line treatment options or beyond for gastroesophageal cancers are limited. TROP2 is a cell surface glycoprotein involved in cancer growth, metastasis, and drug resistance. Sac-TMT (also known as MK-2870/SKB264) is a TROP2-directed ADC developed with a novel linker to a belotecan-derivative topoisomerase I inhibitor. Sac-TMT has shown preliminary efficacy and manageable safety in a phase 1/2, first-in-human study of sac-TMT in patients (pts) with locally advanced unresectable or metastatic solid tumors, including gastric or gastroesophageal junction adenocarcinoma, refractory to available standard therapies (NCT04152499). In the open-label, randomized, phase 3 MK-2870-015 study (NCT06356311), the efficacy and safety of sac-TMT is being evaluated vs TPC as third-line treatment or beyond for pts with metastatic GEA.
Trial design
Eligible pts are ≥18 years of age with histologically or cytologically confirmed advanced unresectable or metastatic GEA. Pts should also have measurable disease per RECIST v1.1 by investigator assessment, have disease progression after ≥2 prior lines of therapy (including a fluoropyrimidine and a platinum doublet), and an ECOG PS of 0 or 1. Approximately 450 pts will be randomly assigned (1:1) to receive sac-TMT 4 mg/kg IV on days 1, 15, and 29 of every 42-day cycle or TPC (TAS-102 [35 mg/m2 PO BID on days 1-5 and 8-12 of every 28-day cycle]; irinotecan [150 mg/m2 IV on days 1 and 15 of every 28-day cycle]; paclitaxel [80 mg/m2 IV on days 1, 8, and 15 of every 28-day cycle]; or docetaxel [75 mg/m2 IV on day 1 of every 21-day cycle]). Both treatment arms will also receive best supportive care. Treatment will continue until disease progression, unacceptable toxicity, or other discontinuation criteria are met. Randomization will be stratified by TROP2 expression, geographic region, and ECOG PS. The primary end point is OS. Secondary end points are PFS, ORR, and DOR (all per RECIST v1.1 by BICR), and safety/tolerability. Enrollment is ongoing.
