Background
Renal cell carcinoma (RCC) comprises 80%-90% of renal malignancies globally, with a 3% incidence rate worldwide. It is among the deadliest urinary system malignancies, primarily treated with surgery.
Methods
We conducted a retrospective study of 3099 patients who underwent robot-assisted laparoscopic surgery for renal tumors at our center from 2014 to 2023. The study analyzed surgical techniques, postoperative pathological findings, and patient prognoses.
Results
From November 2014 to June 2023, our center performed 3099 robot-assisted laparoscopic surgeries for renal tumors. Among these, 1936 (62.47%) were male and 1163 (37.53%) were female. Procedures included partial nephrectomy in 2611 patients (84.25%) and unilateral nephrectomy in 488 patients (15.75%). Pathological findings revealed malignancy in 2491 patients (80.38%) and benign conditions in 608 patients (19.62%). Renal clear cell carcinoma accounted for 2096 cases (67.63%), followed by papillary renal cell carcinoma in 140 cases (4.52%). Grade II clear cell carcinoma was most prevalent according to the WHO/ISUP grading system, with 1184 patients (56.49%).Long-term follow-up (31 to 482 weeks) included approximately 2800 patients. Nineteen patients experienced recurrence (overall rate 0.47% post-partial nephrectomy), and 71 patients developed metastasis (overall rate 2.29%), predominantly to the lung (43.66% of metastatic cases), with other sites including bone, adrenal glands, and abdomen. Fifty-one patients died during follow-up (1.82%), with 13 deaths attributable to renal cancer (0.46%). The disease-free survival rate following robot-assisted laparoscopic surgery for renal tumors was approximately 98.19%.
Conclusions
Robot-assisted laparoscopic surgery for renal tumors demonstrates superior safety and efficacy compared to traditional laparoscopic techniques. Long-term follow-up results indicate significantly lower rates of recurrence, metastasis, and mortality compared to conventional approaches.
