Abstract 465P

Background

The enhanced application of imaging techniques is resulting in the diagnosis of more mEGC patients who are still asymptomatic. The question arises whether in these patients palliative systemic therapy should be started directly or can be postponed. We carried out a Delphi study to gather insights from European experts on when to initiate palliative systemic therapy and to determine if there is agreement on clinical factors that guide decision making.

Methods

Physicians were invited through the EORTC-GI group. An online survey featured 14 scenarios where the decision to commence systemic therapy had been made, but the timing of starting was still open for discussion. The physicians were asked to select their preferred timing: direct (<3 weeks) or postponed. If they opted to postpone it indicated their belief that clinical deterioration would not result in losing the opportune moment for treatment. The standard scenario was a 65 years old male, WHO/ECOG 0 with asymptomatic mEGC, 2 metastases in each lung, HER2 -, PDL1 CPS 2. In every subsequent case one characteristic was modified. To investigate the fortitude of the physicians’ preference for an immediate start, scenarios also included a patient who was motivated to start but preferred to postpone if the physician deemed it judicious. Consensus was defined as =/> 75% agreement, scenarios without consensus were re-evaluated in round 2. Finally, an online meeting was held to discuss the results.

Results

In round 1, 39 physicians participated; in round 2, 33 participated, and 16 attended the online meeting. Consensus was reached to start treatment directly in 12 (86%) scenarios. When patients preferred to postpone, the consensus was to still advise to start palliative systemic treatment immediately in 7 (50%) cases. There were only 2 scenarios (pre-existent WHO/ECOG 2 or 78 years old) in which consensus was reached that treatment could be postponed.

Conclusions

In asymptomatic mEGC, direct start of treatment is preferred by European experts, while always respecting patients’ preferences. Consensus was achieved that treatment could be delayed exclusively for patients who prefer postponement and either have a pre-existent WHO/ECOG performance status of 2 or are of advanced age.
