Background

Colorectal signet ring cell carcinoma (SRCC) is a rare entity with poor prognosis. The aim of the study was to investigate the clinicopathological characteristics and identify the prognostic factors in colorectal cancer containing signet ring cell (CRC-SRC).

Methods

We retrospectively analyzed 98 CRC-SRC patients confirmed by pathology from February 1987 to December 2015 in our institute. Age, gender, clinical characteristics, laboratory findings, pathologic grade and TNM staging were studied to identify prognostic factors associated with the overall survival (OS) and progression-free survival (PFS). Statistical analysis was conducted by SPSS 20.0.

Results

Of the 98 patients in this study, 60 rectal patients accounted for 61.2%. Ninety-five (96.9%) patients were diagnosed as stage III or IV tumors, 76 (77.5%) patients had lymph node metastasis, and 27 (27.5%) patients had distant metastasis. The median survival time was 22.1 months (range, 1-165 months). The 1-, 3-, and 5-year OS rates of the patients were 74.5% (95%CI: 65.9%-83.1%), 44.8% (95%CI: 34.8%-54.8%), and 31.2% (95%CI: 21.4%-41.0%), respectively. Preoperative carcinoembryonic antigen (CEA), N staging, M staging, local relapse were associated with OS (χ2=4.163, 8.320, 11.199, 15.436, P 

Conclusions

Long-term follow-up of colorectal SRCC showed that it was a rare subtype of colorectal cancer with poor outcomes. Furthermore, the dismal prognosis was related to the presence of signet ring cells, rather than the proportion of signet ring cells. Preoperative CEA and distant metastases were hopeful to predict prognosis of CRC-SRC independently.
