Abstract

Background

Standard of care for early stage nasopharyngeal carcinoma is radiotherapy and non metastatic Stage II-IV NPC is concurrent chemo-radiotherapy with or without adjuvant chemotherapy. Yangon General Hospital is a 1500 bedded affiliated teaching hospital of University of Medicine 1 and University of Nursing, Yangon. Radiation oncology unit has two Cobalt 60, two linear accelerators and one HDR brachytherapy machine to provide radiation therapy for cancer patients in Myanmar.

Methods

Clinical data of patients with nasopharyngeal carcinoma treated at YGH radiation oncology unit from January 2015 to December 2015 were reviewed and analyzed.

Results

A total of 101 patients (59 males and 42 females) were registered with Nasopharyngeal tumor; Male: Female ratio is 1.4:1. The youngest was 15yrs and the oldest was 87 yrs (mean = 49.5yrs). Patients under age 40 were 23.76% (n = 24). Different clinical presentations with neck glands involvement in 58.41% (n = 59)of patients, with local extension and brain in 10.89% (n = 11), with distant organs metastasis in 5.94% (n = 6) were seen. Histopathology showed 53.46% Squamous cell carcinoma (n = 54), 14.85% Anaplastic carcinoma (n = 15), 4.95% Lymphoma (n = 5) and 7.92% were others (n = 8). After registered for radiotherapy treatment 40.59% (n = 41) of patients did not come up for radiotherapy. 3.9% (n = 4) of patients lost follow up after 2 cycles of chemotherapy. Concurrent chemo radiotherapy using cisplatin was done in all patients received radiotherapy and 42.57% (n = 43) received radiotherapy with Cobalt 60 and 12.87% (n = 13) with 3DCRT.

Conclusions

Nasopharyngeal cancer is one of the ten commonest cancers registered in 2015 in Yangon General Hospital. Majority of patients were presented with neck glands involvement and relatively high numbers of young patients were seen. Due to stigma about radiation treatment in general public and long waiting list, nearly half of registered patients did not attend for proper treatment. Thus, accessibility of radiotherapy facilities should be improved. Besides, health education for public awareness for nasopharyngeal carcinoma and radiation treatment should be promoted to get early diagnosis and proper treatment.
