In the absence of distant metastases, lymph node staging becomes the pivotal factor in determining treatment and prognosis for lung cancer patients. The sensitivity of PET-CT for nodal staging remains sub-optimal. Using a different SUV ‘cut-off’ value to define a positive lymph node (traditionally 2.5) or using the ratio of the lymph node SUV compared to the primary tumour SUV may provide more clinically relevant information.
